In William Gibson's Neuromancer (and other cyberpunk foundations) people surfing the Net could be killed off (in "real" life) by maleficient agents they encountered (especially when hacking into places they shouldn't be).

Now, there is an online fantasy style wargame called World of Warcraft (which claims more than 4 million regular, online players) here and here.

To add a bit of "spice", the creators included a creature whose poisoned blood kills off protagonists when it spatters on them. However, hackers have got in the works and the poisoned blood has mutated and is running amok (still only in the gaming realm). According to the BBC here.
