If you would prefer to fill out the forms by hand, the following forms are Adobe Acrobat files that can be downloaded to your computer and printed for your convenience in completing the forms. To downlaod a free copy of the Adobe Acrobat Reader please go to www.adobe.com. I ask that you complete these and bring them with you to your first appointment. This evaluation session will last for about 60 minutes. If you have any questions about any of the forms I will be happy to address them with you at our first meeting.
If you are going to be using insurance, please be sure you check with your insurance company as soon as possible and ask if preauthorization is required prior to our first meeting. Most insurance companies do not require this, but a few do and failure to abide by their rules may result in lower coverage for you.
Please carefully review my office policies and procedures and be sure to clarify any questions or concerns you have about them. It is very important that you keep your first appointment, or notify me at least 24 hours in advance if you cannot make it. It is my policy not to reschedule patients who fail to keep their first appointment or who cancel with less than 24 hours advance notice.
