Transportation Forms for Year Round IEPs

As you do your Annual Case Conference, fill out the Transportation Form for NEXT YEAR (beginning August 18, 2010). Add the completed form to your current IEP, but do not fax downtown right now. Keep a folder of all of your Transportation Forms, and you will fax them all together at the end of this school year.

iStar

ISTAR data will NOT be collected by the state this fall. This year's assessment window will be March 1 - April 30, 2010.

You can combine the dates for two school years within one Service box if you specify both initiation dates and both Duration dates. Click here for instructions.

Year Round IEPs

You will hold an annual case conference for each student anytime in the month of their birthday, regardless of which date it occurs on. When you finalize the IEP, enter the LAST day of the birth month as the initiation date of that IEP, irregardless of the date of the conference. IStart will then automatically carry that date forward 1 year and enter the ending date for you. So, if a student's birthdate is 9/1/2009, you will enter 9/30/2009 as the initiation date, and iStart will enter 9/30/2010 as the ending date. You must then hold the NEXT ACC for that student in September, before 9/30/ 2010.

If an initial is held for a student, set the initation date as the date of the conference, and the ending date as the LAST day of the month of the birthdate. You must then hold an ACC within the month of the birthdate. The effective dates for the ACC would then be as stated above.

For summer birthdates: All summer birthdates (May, June, July and August) must be held within the month of April. This change takes place as of September 1 2009, so students with current August birthdates can be done in April for this year.

You may select "Revise the current IE"P for each ACC. This will carry forward information from the past IEP. For middle and high school students, you may need to enter course information for this year, and list them separately for next year. You will designate each course or service by beginning and ending dates.

Quarterly Reports

New information has been posted on how to keep data on your goals and use it to complete your Quarterly Reports. Go to Goals --> Quarterly Reports to check it out.

If retesting is requested at a case conference, we will no longer be using the Re-evaluation worksheet. Use the pink forms, fill out your portion, and give it to the psychologist or related service provider within 3 days of the conference.

If the student is newly enrolled, and you do not have information confiming the disability, include the following statment in the text box under the Eligibilty Tab-->Determine Eligibility subtab: "Student was determined to be eligible but confirmation of eligibility under Article 7 is pending the receipt and review of records and/or further evaluation."
