HELP!!! I NEED GLASSES!!!
1. If the student has Title 19 there are three Optometrists in Sioux City at this time that will accept Title 19 for BOTH the exam and glasses:

2. If the student does not have Title 19, do they have any other vision insurance? Has the family applied for Hawk-I? Hawk-I will pay for glasses and the vision exam. 1-800-257-8563 (TDD 1-888-422-2319) www.hawk-i.org
3. If neither of the above work, consider applying to Prevent Blindness in Iowa by obtaining their Vision Service Plan-Sight for Students Application form: Prevent Blindness in Iowa, 1111 Ninth Street, Suite 250, Des Moines, Iowa 50314-2585. 515-244-4341 or 800/329-8728. Make sure to make some extra copies for your school. Note that the form states that a parent or guardian’s social security number may be substituted instead of the child’s if needed. This application can then be mailed or faxed to the DesMoines office.
4. If all else fails and the student does not have Title 19 and for example doesn’t have a social security number here are some other resources to try:
Both Super Walmart’s have a program called Project Insight. They will fund 20 students per year each for glasses and an eye exam. They try to do one student per school. (Total of 40 students per year) They would appreciate the referral to come from someone at the school instead of the parent.

*The Salvation Army has a program called Project Eye Care. They will fund 5 students per year. They would also appreciate someone from the school to make the referral. Please contact Valerie Jones at: Salvation Army 510 Bluff Street 255-8836
*Sioux City Noon Lions Club — An application can be obtained through your school nurse's office at each K-12 Sioux City Community School.
