General Affidavit

 

State of New York County of Otsego

 

In claim No. 177629 of William Lunn of Co. K of the 121st Regt. of NY Vols. Personally appeared before the undersigned duly authorized to administer oaths within and for said County, William Lunn aged 46 years whose P.O. is Garrattsville, County of Otsego State of NY, who being duly sworn, states in relation to said claim as follows to-wit;

I was treated for Malaria by Dr. Valentine and Holt both now dead they treated me while I was at the Regimental Hospital; White Oak Chuch Va As both those Doctors are now dead I am unable to get their affidavits. I was treated at Lincoln Hospital for what was called intermittent fever but never knew the Doctors name My Home Physicians who have treated me since my return from the Army are all dead except Dr. Chambers who now resides at Hartwick Otsego Co NY

The Assisstant Surgeon at the Hospital I cannot remember now do not know his name

Sworn to and subscribed before me on the 12th day of June 1888, and I hereby certify that the contents of this affidavit was fully made known to the affiant before signing and I have no interest in this claim or its prosecution
