Sport Physicals for Spring 2008 will be offered on:

• Tuesday
• February 5th and 12th
• 2:30 - 4:00 P. M.
• North Building
• Room 107

The Sports Physicals forms have been revised as follows:
        Part A:

• Page 1 - 3 MUST BE COMPLETED & SIGNED BY A PARENT/GUARDIAN prior to the medical visit.

REMEMBER TO BRING THE ENTIRE PACKET WITH YOU THE DAY OF YOUR PHYSICAL EXAMINATION.
