SPORTS PHYSICALS
Sport Physicals for Spring 2009 will be offered on:
Tuesday
January 20 , February 3, and 10
2:30 - 4:00 P. M.
North Building
Room 107
The Sports Physicals forms have been revised as follows:
Part A:
Page 1 - 3 MUST BE COMPLETED & SIGNED BY A PARENT/GUARDIAN prior to the medical visit.
REMEMBER TO BRING THE ENTIRE PACKET WITH YOU THE DAY OF YOUR PHYSICAL EXAMINATION.
