Fall Sports Information and Start Dates
Each athlete MUST have a practice permit before he/she can go out for a sport. All necessary forms can be picked up in the Activities Office or the Main Office (in the summer) at Park High School. The following two (2) items must be completed to receive a practice permit:
1. WIAA PHYSICAL/WAIVER CARD (or Alternate Year Card) Athletes must have a green physical card on file for the coming school year – OR - Use the alternate year card if the physical card was submitted this school year. The Racine high schools WILL NOT accept middle school physical cards for high school participation; all incoming 9th grade athletes must have the green physical card. Physical cards are good for two school years (if taken after April 1, 2008) and kept on file in the Activities Office. Athletes or parents can check with this office to see when the card expires. This should be done well before school ends this year in case a doctor appointment is necessary. The Insurance & Athletic Code waiver form is on the back of the green physical or alternate year cards. Family insurance coverage information should be included on these cards. The district offers an insurance plan which may be purchased if there is not sufficient family insurance. Your signature on the WAIVER FORM indicates your insurance status and indicates that both the athlete and parent have read the District Athletic Code.
2. USER FEE The User Fee and family maximum are set by the school board each year; currently $60 per sport, $180 maximum per family. Checks should be made payable to Park High School. User fees are ONLY refundable if the athlete is cut from the sport; refunds must be requested from the Activities Office.
The Activities Office, Room 147, (619-4443) will be open to issue permits as listed below. Athletes are expected to be ready for practice at the designated time, not waiting in line for a permit.

Mail In Option-Due August 1st: Again this year will be the opportunity for athletes who are confident that their card is complete to mail the WIAA card and the User Fee (check or money orders only) to Park HS - Attention Activities Office. The Practice Permit & receipt will then be given directly to the coach by our office. The sub schools and main office can not issue receipts or permits; please do not ask them to do so. Forms that are mailed must be received at Park HS by Friday, August 1st. Please indicate the athlete’s fall sport (i.e. Football, G Swim, etc). Only sign up for a fall sport at this time.
