The Antioch ESPACE Academy is a local and regional center for standards-based astronomy education at all grade levels.

The Academy provides students with a motivational venue for learning about science through research or outreach oriented projects based on instruction in earth science, astronomy and space science.

Application Forms Table
Download and print all of these forms, fill them out and turn in as a packet to the House 2 Office.Blank copies are available at the House 2 Office and from ESPACE Academy teachers.
For 2004, the forms are due on February 20.
Form Checklist This checklist is also downloadable as an Adobe Acrobat document
Parent Questionnaire On this form parents give permission for you to be in the academy and give contact information and the answer to a few questions.
Student Contract Students agree to abide by the rules of the academy.
Course Plan Students plan all four years of high school and show how the academy fits in. For older students, courses completed are recorded.
Student Interest Students explain why they want to be in the academy.
Recommendation Two recommendations from teachers are required for admittance.
Internet Permission The internet will be used extensively in the academy so permission to use the internet at school is needed. You do not need this form if you have internet access permission granted already.
Publishing Release Student work may be published in journals, web sites, or other venues. This gives us permission to use student work. Please, if you don't mind, give us indefinite permission so we can use your work for future students as examples.
