Dr. Zito escapes after a sanity hearing where he is defended by his psychiatrist Dr. Skinner to no avail. Having lost interest in Lieutenant Murphy he now targets MacGyver as his enemy, leaving a trail of "by the numbers" clues intended to give MacGyver a crash course in evil. MacGyver discovers that Zito's scheme spins around the legend of the Three Fates. He receives a phone call from Zito threatening his friends, loved ones, and co-workers. Pete is temporarily held hostage and forced to wear a helmet used to prevent a violent patient from biting off his tongue. After Pete is freed, Murphy suffers fractures in an explosion at MacGyver's home. Lastly, he must rescue Dr. Skinner, held at an asylum which was shut down forty years ago. MacGyver confronts Zito in the bell tower of the asylum, and he leaves him bound with ropes and suspended in the shaft beneath the ringing bell. In possession of the key, he unlocks the door to the room where Dr. Skinner is held and saves her from electrocution.
Dr. Zito: Do you actuallly think you're qualified to judge what is evil?
MacGyver: I just call it like I see it.
