In Chicago MacGyver comes to see rock star Roxie Yates at a video shoot of "Higher Life", part of the R.A.D. campaign. The video is set in a club scene with a huge skull backdrop and a couple dropping into a tank alongside, sinking to the depths of drug influence. She and MacGyver go back to her penthouse but must climb up the elevator shaft when the elevator unexpectedly stops. Roxie says that her twin sister Carla has been trying to murder her, and there have been other incidents. On the contrary, Carla tells him later that Roxie recently had a mental breakdown. As he looks closer at Carla's latest painting, he sees the two mountain climbing, and Roxie, hanging in mid-air, is cutting the rope. He goes to Pine Valley Hospital to talk with Dr. Farrell, who tells him that Carla was the patient, not Roxie. Carla has been posing as her sister since her death. Returning to the empty video shoot area, he finds Carla standing in the eye of the skull. When he comes closer, she knocks him out and pushes him down the chute into the water tank. He gets out and comes to the dressing room just as she is about to kill herself. Guilty that Roxie died to save her on the mountain she is in denial of Roxie's death. MacGyver visits her later, safely back in the hospital and recovering from her grief.
Roxie: Do you remember all the fun we had?
MacGyver: Yeah, I do.
Roxie: That smile. You have the nicest smile of anybody I've ever known.
